Case Withdrawal

No practitioner shall abandon a patient. In the event a practitioner must withdraw from a case, he or she shall give reasonable written notice to the patient, patient’s family or patient’s caregivers so that they may find an alternate practitioner to take over the patient’s care. In addition, except under emergency circumstances, any practitioner withdrawing from a patient’s case shall upon written request make available to the patient, family, or caregivers, and to any practitioner or provider caring for the patient, all records, results, and other medical record information related to the patient that are under the practitioner’s possession, control, or custody.
