Hospice care is an option provided by an interdisciplinary team of the patient’s choice. The team must consist of a physician, nurse, social worker, trained volunteer, and the patient and the patient’s family. The members of the interdisciplinary team must be licensed and qualified professionals. One member of the team must be designated as the patient care coordinator. The team is responsible for performing an initial assessment of the patient and developing the care plan for each patient. The patient care coordinator is responsible for implementation of the care plan and ongoing review of the plan.
