“Advance directive registry under the Agency of Human Services regulations”

The Vermont Advance Directive Registry (VADR) is a secure, online database to which individuals may submit an advance directive or an advance directive locator at no charge. VADR is a voluntary database.  VADR serves only as a repository of information and documents and will not evaluate the validity of documents except to determine whether the registration Agreement is complete. Medical providers and facilities may access the VADR system for information on a person’s advance directive.
