“Vermont Blueprint for Health under the Agency of Human Services regulations”

The Blueprint for Health is Vermont’s program for integrating a system of health care for patients, improving the health of the overall population, and improving control over health care costs by promoting health maintenance, prevention and care coordination and management.  The Department of Vermont Health access must periodically publish a Blueprint for Health Manual, which may be changed through the public comment process.

The Blueprint director must oversee development and implementation of the Blueprint for Health.  The Health Care Reform director must support development and implementation of the Blueprint for Health with the Blueprint director.

There must be three groups serving in an advisory capacity to the Blueprint director:

  • The blueprint executive committee which must advise on the blueprints or strategic planning.
  • The expansion design and evaluation committee which must advise under blue prints expanded implementation.
  • The payer implementation work group which must advise and the technical details of payment implementation and modification.

Physicians, nurse practitioners, or physician assistant practice sites providing general primary care services, family medicine, internal medicine, obstetrics and gynecology, or podiatric medicine are eligible to participate as a Blueprint medical home.

Community health teams are multi-disciplinary teams developed to meet the specific needs of each community.  Health care professionals participating on a community health team must:

  • Collaborate with other health care professionals;
  • Support a health care professional or practice which operates as a medical home;
  • Provide care management support when a patient who has to a new setting for care; and
  • Work with state agencies on care coordination issues for Medicaid and Medicare beneficiaries and supporting public health initiatives.

Health insurers must participate in the Blueprint for Health as a condition of doing business in Vermont.

Hospitals must participate in the Blueprint for Health by establishing and/or maintaining connectivity to the state’s health information exchange network.
