Maintenance of Patient Records by Opticians

Practitioners must maintain contact lens records for a minimum of 5 years.  The records must include at least the following, which adequately reflects the level of care provided by the practitioners: the initial written prescription, dioptric power, lens material, brand name, base curve, diameter, color, thickness, secondary/peripheral curves, variable curves, suggested wearing schedule and care regimen.  If the optician is fitting contact lenses, the prescriber’s records must additionally include documentation of written advisement to the patient of the need to obtain a follow-up evaluation.  Finally, all prescribers’ records must include documentation of contraindications which would prohibit contact lens wear and documentation that the contraindications were explained to the patient by the prescriber, and explanatory notation of the reasons why a prescription has an expiration date of less than two years and documentation that the reasons were explained to the patient at the time of the eye examination.
