Retention of Records, A.R.S. § 12-2297

“Retention of records”
 
Arizona health care providers must retain medical records for at least 6 years after they last provide care to an adult patient. Providers must maintain records regarding minor patients for the later date of either 3 years after the minor turns 18 or 6 years after the last date of service. Source data must be retained for at least 6 years following its collection, but may be maintaned apart from the medical record.
Providers must ensure their compliance with these recordkeeping requirements even if they retire or sell their practice. Nursing care institutions must also retain records for 6 years following the discharge of an adult patient or the later of 3 years following a minor’s 18th birthday or 6 years after a minor’s discharge.
