Required Annual Reports for Licensed Health Facilities

A licensed health facility must provide to the Office of Statewide Health Planning and Development the following: 

  • A balance sheet for general (restricted) fund
  • A balance sheet for restricted funds
  • A statement for changes in equity for restricted and unrestricted funds
  • A statement of income and expense
  • A statement for cashflows of unrestricted funds
  • A cost finding report
  • A detailed statistical report
  • Data required for Medicaid (Medi-cal) reimbursement
  • A statement detailing patient revenue by payor
  • Other reports required by the Office
