Reports to department and coroner - 35 Pa. Stat. Ann. § 10225.702

The local provider of protective services must transmit to the department of aging written reports involving sexual abuse, serious physical injury, serious bodily injury or suspicious death of a recipient of services from any of the following facilities:

  •         Domiciliary care home;
  •         Home health care agency;
  •         Long-term care nursing facility;
  •         Older adult daily living center; or
  •         A personal care home.

Reports must include the following information:

  •         The name and address of the alleged victim;
  •         Where the suspected abuse occurred;
  •         The age and sex of the alleged perpetrator and victim;
  •         The nature and extent of the suspected abuse, including any evidence of prior abuse;
  •         The name and relationship of the individual responsible for causing the alleged abuse to the victim, if known, and any evidence of prior abuse by that individual;
  •         The source of the report;
  •         The individual making the report and where that individual can be reached;
  •         The actions taken by the reporting source; and
  •         Any other information which the department may require by regulation.
