Release of Limited Information on Specific Patient Unless Written Request by Patient to Prohibit – Cal. Civ. Code § 56.16

If asked about a specific patient, a general acute care hospital can release the patient’s name, address, age, sex, a general description of the reason for treatment, general nature of the injury, and general condition of the patient, unless the patient provides a written request telling the hospital not to disclose any of the above information.
