All facilities that provide outpatient medical services must conduct ongoing, comprehensive self-assessments. Such assessments must include the condition of patients and services rendered, patient records, administration, staff training, and policies and procedures. When policies are identified, the facility must act as soon as possible to avoid risks to patients. Such actions includes changes in policies, staffing changes, additional staff education, and changes in equipment.
