Patient Records-N.M. Code R. § 7.11.2.24

All facilities that provide outpatient medical services must maintain medical records for each patient. All records must be legible and accurate, and must include the following information:

  • Patient identification;
  • Medical history;
  • Allergies and abnormal drug reactions;
  • Discharge diagnoses;
  • Patient consent forms, if applicable;
  • Assessment of health status and health care needs;
  • Reports of physical examinations;
  • Nursing notes; and
  • Medication charts.
