Patient Profile Records
 
Requires pharmacies to maintain a profile record for every patient to whom they dispense prescriptions. Specifies the information that pharmacists must document for each new prescription (e.g., name of individual prescribed the drug, patient date of birth, patient’s medication history, etc.). Requires pharmacists to make a reasonable effort to obtain information from the patient relevant to prospective drug review (e.g., known allergies, use of over the counter mediations, chronic conditions, etc.).
