Patient Consent
Permits patients or their parents to opt out of participating in the WY IIS by completing a form available on the Wyoming Department of Health’s website. Prohibits the Department from maintaining individually identifiable information regarding a person who opts out, but requires to Department to maintain immunization information in aggregate form.
