“Reporting of communicable diseases under the division of public health services regulations”

Any physician, other health care provider, or laboratory must report cases of a reportable disease to the Department of Health and Human Services. Physician or health care provider’s reports must contain information on the patient, the name of the disease, and the patient’s medical history and treatment. Laboratory reports must contain test results, the patient’s information, and health care provider’s information.
