Health care home reporting requirements
An annual report of the implementation and administration of the health care homes must be submitted to the legislature. There must also be evaluation reports of the health care home model three and five years after implementation. The evaluation report should include the number of state health care beneficiaries enrolled in health care homes, key characteristics of enrollees, including number and type of chronic conditions, and race, ethnicity and language. The number and locations of the health care homes must be identified, along with their performance and quality, preventative care measures, payment arrangements, estimated impact on health care disparities, and estimated cost savings from implementing the health care home model.
