Mental Health information Disclosure Prohibited -- exceptions -- record of disclosure. 
 
Prohibits mental health professionals, mental health facilities, or employees or agents of such professionals or facilities from disclosing mental health information unless disclosure is for care coordination purposes or occurs in accordance with state or federal law. Requires persons disclosing mental health information in permissible circumstances to document the disclosure in the individual’s mental health record. Requires persons to include a notice in disclosures that additional disclosures are only permissible in accordance with state or federal law or with the authorization of the individual whose information they have received and that violations may result in civil or criminal liability. Permits the transfer of an individual’s mental health information to other professionals or facilities upon the request of the individual or their authorized representative.
