Medication therapy reviewed monthly
Requires pharmacists to conduct reviews of a nursing facility resident’s drug regimen at least once a month. The reviews should examine the resident’s diagnosis, lab finding, dietary considerations, and current drug regimen. Requires pharmacists to report potential irregularities and recommendations to the resident’s health care providers (e.g., attending physician, nurse practitioner, etc.). Requires nursing facilities to retain the pharmacists report and place a copy of it in the resident’s medical record.
