Medical and Health Report for Live Births – Cal. Health & Safety Code § 102450

“Medical and health report; completion and submission for registration; filing”

 

For every registration of live birth sent to the local registrar’s office, a hospital administrator or representative must submit a medical and health report to the State Registrar.

 

For live births that occur outside of a hospital, the medical and health report shall be completed and submitted by the local registrar when the local registrar receives the certificate of live birth for registration.
