Establishes abortion reporting requirements. Authorizes providers who perform less than five abortions per year to report via a paper form; otherwise requires abortion reporting to occur electronically. Specifies the information that must be included in the reports (e.g., patient age, date of the abortion, unique patient identifier, etc.).
