Kan. Admin. Regs. § 28-39-151, Resident assessment

Requires nursing facilities to conduct an assessment of residents upon their admission and periodically during their time in the facility. Identifies the elements that the assessment must include (e.g., medical history, cognitive status, discharge potential, etc.). Requires the facility to use the assessment to develop a care of plan for the resident and for this plan to be developed by an interdisciplinary team, the resident, and resident’s family and legal representatives. Requires facilities, when discharging a resident, to create a discharge summary and make this summary available to agencies and other persons with the resident’s consent. Requires facilities to create a post-discharge plan to help the resident transition to life outside of the facility.
