Documentation Standards
 
Establishes standards for documentation of DD waiver services, medical records, and financial records. Requires providers to complete all documentation before submitting a claim. Permits providers to document services either in writing or electronically. Requires electronic documentation to include electronic signatures and automatic date stamps. Specifies the information that each page of written documentation must include (e.g., participant name, billing code of service provided, etc.).
