Permits prescribers and dispensers to request patient information from the drug monitoring program’s central repository by submitting a signed request form that includes information such as the identity of the patient, the reason for the request, and a statement that the prescriber or dispenser has a relationship with the patient. Request forms must be labeled confidential and may be submitted by mail, fax, or electronically.
