Hospitals must complete form MA 336 (Financial Report for Hospital and Hospital-Health Care Complex Under the Medical Assistance Program of the Department of Public Welfare) and annually submit it to the department of welfare’s office of medical assistance.  If the hospital participates in Medicare, a complete copy of its Medicare cost report must also be submitted to the department.  The hospital’s cost report must include all information necessary for the proper determination of costs payable under the program, including financial records and statistical data.
