Every person attending an adult day service program must have a comprehensive written assessment.  It must be given starting no later than the first day of participation, and must be finished within 30 days.  The program may use either an assessment tool provided by the Department of Health and Human Services, or its own assessment that meets criteria set forth by the Department.  All consumer records must be kept confidential in controlled access files.  The records must be kept at the program site or central office for a minimum of 5 years after the consumer leaves the program.
