Medical Record Services

Licensed Alabama hospitals must assign responsibility for medical record administration to a  “medical records service.” Hospitals must maintain medical records regarding an adult patient for at least five years while medical records regarding children must be maintained for a minimum of five years after the child reaches the age of eighteen. Hospitals must comply with standards regarding the content of medical records. These standards include items such as the patient’s medical history, diagnosis, and provision of informed consent.

Hospitals must establish a process to safeguard the confidentiality of patient records. Hospitals must limit access to patient records to authorized individuals and may only release medical records pursuant to court orders or in compliance with federal or state laws.
