“Recordkeeping”
Requiresschools maintain records sufficient to demonstrate compliance with 956 Mass. Code Regs. 8.00, including:
These records must include: a copy of the School's Student Health Insurance Program offered to Students; a copy of the School's Student Health Insurance Program's Evidence of Coverage; all informational materials developed by or for the School to describe its Student Health Insurance Program; the Student's obligations to participate in his or her School's Student Health Insurance Program; and all written waiver requests.
Such records must be maintained for at least three years.
