b.) Consent Invalid

A written consent form is invalid if:

  • It has expired;77
  • Substantially fails to conform to any of the requirements set forth above;78
  • Is known to have been revoked;79 or

Is known, or by reasonable effort could be known, by the record-holder to be materially false.80
