Care Coordination/Care Management in Vermont

Vermont’s Blueprint for Health is an initiative focused on integrating the state’s healthcare system, improving public health, and decreasing health care costs.1 The Department of Vermont Health Access is responsible for developing and implementing the state’s Blueprint for Health.2 The Blueprint must focus on chronic care management, coordination of health care services, payment reforms for health insurers, Medicaid, and Medicare, and development of medical homes and community health teams.3 The Blueprint for Health extends to the state’s mental health system.4 Health insurers and Vermont’s Catamount Health must comply with the Blueprint for Health.5 Hospitals are required to participate in Vermont’s health information exchange network to comply with the Blueprint for Health.6 The director of the blueprint must annually report to the state legislature on the progress of implementing the Blueprint.7

The Green Mountain Care board is responsible for payment reform and delivery system reform as part of the state’s health care reform efforts.8 The director of payment reform from the Department of Vermont Health Access must develop the payment reform pilot projects according to the policies established by the board.9 Health insurers must participate in the development and implementation of the payment reform pilot projects.10
