Care Coordination/Care Management in New Hampshire

New Hampshire has various laws to enhance care coordination and management. The state requires its Medicaid and CHIP programs to be operated through a managed care model, which includes accountable care organizations and medical homes.1

Health care providers must ensure that patients receive individualized treatment plans that may be developed by interdisciplinary teams.2 They must also ensure that patients have the ability to transition from one health care provider to another in a coordinated manner.3 Patients have the right to be informed prior to a transfer or discharge.4
