Care Coordination/Care Management in Florida

Rural health networks are responsible for establishing agreements with health care providers to manage care for patients.1

The Florida Legislature established the following programs:

  • A statewide comprehensive and coordinated interdisciplinary program of early hearing impairment screening, identification, and follow-up care for newborns;2
  • A comprehensive program dealing with the early detection and prevention of chronic diseases is required to make knowledge and therapy available to everyone to reduce death and illness3

The Children’s Medical Services program must provide children with special health care needs a family-centered, comprehensive, and coordinated statewide managed system of care that links community-based health care with multidisciplinary, regional and tertiary pediatric specialty care.4

Medicaid managed care plans must have well-defined programs for recognizing patient-centered medical homes and providing for increased compensation recognized medical homes.5 The Agency for Health Care Administration and Department of Elderly Affairs must seek a federal waiver to implement a Medicaid home and community-based waiver targeted to persons with Alzheimer’s disease.6

Patients have the right to participate in treatment and discharge planning.7

Nursing homes and hospices must provide interdisciplinary medical care for patients.8

If a patient is transferred to another health care provider, the patient’s records should be transferred as well.9
