Primary care health homes — Chronic care management — Findings — Intent

The Washington legislature finds that primary care health homes can constrain rapidly increasing health care costs, and can be used to manage chronic disease, improve health outcomes and reduce costs. 

The legislature (1) encourages primary care health homes to collaborate with other providers currently outside of the traditional insurance model; (2) believes that primary care providers, long-term care workers, and long-term care service providers must coordinate in order to successfully provide long-term care management services; and (3) encourages primary care providers to coordinate oral health services with dental providers.
