Content of Hospice Care Center Services Client Records

Hospice Care Centers must maintain a current record for each patient.  The record must be accessible, in an integrated document in the center’s office site for review by appropriate personnel, and in chronological order.  Additionally, the record must be kept confidential.  The content of the record must include the patient’s name, age, current address, phone number, consent for care, payment source, initial assessment, plan of care, and any changes in the patient’s condition.  All records should be kept for at least 3 years following the date of termination for adults, and either 3 years after attaining the age of 18 or 5 years following discharge, whichever is longer, for minors.  The records must be stored to prevent unauthorized use, access or disclosure.  =
