W. Va. Code St. R. § 64-12-7 - Paramedical Services

West Virginia’s “Paramedical Services” Rule establishes standards for licensed hospitals’ pharmacy services, dietetic services, and infection control procedures. The Rule requires licensed hospitals to establish a “Medical Records Department and Information System” that is “sufficient to support the maintenance of patient records….and quality improvement activities.” Hospitals must maintain their records for a minimum of five years and implement procedures for protecting the confidentiality of patient records.” The Rule establishes the minimum contents for each inpatient medical records. Required items include: (1) diagnosis information; (2) admission and discharge dates; (3) laboratory results; and (4) discharge planning information.
