“Blueprint for health under the human services law”
Within 5 years following the implementation of Green Mountain Care, each individual enrolled in Green Mountain Care must have a primary health care professional who is involved with the Blueprint for Health. Green Mountain Care must be integrated with the Blueprint for Health.
