Transfer of Care
Permits the transfer of patients to another mental health facility upon completion of a transfer planning conference. Requires facilities to give an individual at least 24 hours written notice of the transfer and provide notice to two persons identified by the individual. Requires facilities to document the provision of notice and the arrangements for transfer in the patient’s clinical record.
