Reporting Requirements for Food Poisoning – N.Y. Comp. Codes R. & Regs. tit. 10, § 2.15

Every physician, visiting nurse, public health nurse, or any other person having knowledge of an illness believed to be caused by spoiled or poisonous food must report the occurrence immediately.  The report may be made by telephone, fax, or other electronic communication to the city, county or district health officer in whose jurisdiction it occurred.
