Provision of services – Ohio Rev. Code Ann. § 3712.06

Hospice programs must comply with the following requirements:

• Provide a planned and continuous hospice care program; 
• Establish an interdisciplinary plan of care for each patient and his family that meets the following requirements:
  • The plan must be coordinated by one designated individual;
  • The plan must address maintenance of patient-family participation in decision-making; and 
  • The plan must be periodically reviewed by the patient’s attending physician and his interdisciplinary team.
• Have an interdisciplinary team that provides or supervises the provision of care and establishes policies governing the provision of the care; and
• Maintain central clinical records on all patients under its care.

Hospice program providers may contract with hospitals, nursing homes and home health agencies to furnish component(s) of the program; such contracts must include the following provisions:

• The contractor must receive a copy of the patient’s interdisciplinary plan of care;
• The contractor must continue the regimen established in the plan of care;
• The contractor must enter all furnished care, treatment and services into the patient’s medical record;
• The designated coordinator of the interdisciplinary team must ensure the contractor’s conformance with the established plan of care; and
• The medical record and discharge summary maintained by the contractor will be retained as part of the patient’s medical record.
