Outpatient psychiatric clinics and outpatient psychiatric partial hospitalization facilities must include the following information in the medical records of medical assistance patients receiving services:
- The treatment plan, which includes treatment plan goals, services to be provided to the patient in the facility or through referral and the persons to directly provide each service; and
- Progress notes, including the frequency and duration of each service provided.
