End stage renal disease dialysis centers must inform clients or their legal representatives of their rights and responsibilities and obtain written confirmation acknowledging receipt of the information. End stage renal disease dialysis centers must have an individualized care plan for every client that is developed by interdisciplinary teams. They must also obtain documentation of informed consent for all treatments or release of information. They are prohibited from using chemical or physical restraints except as permitted by law.
