N.C. Gen. Stat. § 90-411 - Record copy fee

These requirements apply to any person licensed or certified to practice a health care profession or occupation in North Carolina, any licensed health care, mental health or substance abuse treatment facility, and any representative or agent of such persons or facilities. 

If a patient or his or her designated representative requests a copy of the patient's medical records for use in a liability claim for personal injury (other than for a worker's compensation claim) or in a claim for social security disability, a health care provider may charge a reasonable fee to cover the costs of searching for, handling, copying, and mailing such records of up to $0.75 per page for the first 25 pages, $0.50 for pages 26 - 100, and $0.25 for pages 101 and beyond. The provider may set a minimum fee of up to $10, including copying costs.  If the patient or his or her designated representative requests a narrative summary of the patient's medical record, a physician may charge a reasonable professional fee for the review and preparation of such summary. 

These fees do not apply to requests for medical records by the Department of Health and Human Services Disability Determination Services made on behalf of an applicant for social security or supplemental security income disability.
