Any health care provider that immunizes a child must present the accompanying adult with a certificate of immunization. The certificate must list which immunizations were provided, the doses given, the dates when administered, and if additional immunizations are needed. Physicians must inform the parents about their right to object to the reporting of the immunization to the department. If no objection is made, the physician must report to the department each immunization provided. If a physician complies in good faith with the reporting and objection requirements, the physician will not be liable in a civil action.
