Revocation of order

 

If a patient revokes their do-not-resuscitate order, a physician must immediately place the revocation and a writing that describes the reason for the revocation in the patient’s medical record. If the patient does not revoke in writing, the individual hearing the revocation must describe the circumstances of the revocation in a signed writing and deliver it to the attending physician. Upon revocation, the physician must write “Void” on the order, cut the patient’s do-not-resuscitate bracelet, and make the revocation part of the patient’s medical record.
