Ind. Code Ann. § 12-12-9-1 - Reporting of blind or visually impaired persons by physicians or optometrists

This law requires that licensed physicians and optometrists report the name, age, and address of each person diagnosed as being blind or having a visual impairment that would interfere with a person’s functioning in school/employment/daily living to the officer of the secretary within 10 days of making the diagnosis.
