Florida Administrative Code § 64B14-7.002

“Patient records under the Department of Health regulations”

Orthotists and prosthetists must maintain patient records which must include:

  • The patient name;
  • The location and dates of all treatment, evaluation, or consultation;
  • The identity of each provider who treated the patient;
  • The name of the prescribing physician;
  • All prescriptions relevant to the patient; and
  • A treatment or service plan.

Orthotists and prosthetists may charge a fee for providing patients with copies of records, which must be $1 per page for the first 25 pages and $0.50 per page for every page after 25 pages. In addition, the actual cost of postage may be added.
