“Plan of care or clients under the Agency for Health Care Administration regulations”
Home health agencies must establish a plan of care for every patient and include it in clinical records.
Home health agencies must inform patients or the patient’s caregiver or guardian that the patient has the right to:
- Be informed of the plan of care;
- Participate in the development of the plan of care; and
- A copy of the plan of care.
