“Health information management under the Agency for Health Care Administration regulations”
Every hospital must maintain medical records for every patient which must include a patient’s medical history, individualized treatment plan, evidence of appropriate informed consent, and discharge summary. Patient records are privileged and confidential and may not be disclosed without the patient’s consent unless permitted by law.
