Discharge
Specifies the circumstances in which the Wyoming Life Resource Center may discharge a client (e.g., the individual no longer needs services, the client would be better served elsewhere, etc.). Requires the Center to create a post-discharge plan of care and a summary of the client’s health and development upon discharge and submit this summary to authorized persons or agencies upon receiving the client’s consent.
