Connecticut home-care program for the elderly - Conn. Gen. Stat. § 17b-342

The Commissioner of Social Services must manage “the Connecticut home-care program for the elderly” (“Program”) as a means of preventing the institutionalization of elderly persons that are eligible for or receiving medical assistance. The Program must offer services such as care management, occupational therapy, and adult daycare if such services are not offered through the state Medicaid plan.

The Department of Social Services (“Department”) may require elderly persons or their representatives to disclose “financial, social, and medical information” in order to participate in the program. Long-term care facilities must provide the Department “with the names and addresses of all applicants for admission.” Information obtained by the department is confidential and may not be disclosed.
