Access to Patient Medical Records
 
Requires health care entities to provide patients or their designated representatives access to the patient’s medical record. Requires patients to sign and date both their records request and sign an acknowledgment that they have inspected their records. Prohibits facilities from charging patients for access. Permits an attending health care provider to withhold portions of the patient record regarding psychiatric or psychological care or doctor’s notes if they believe that viewing these portions would adversely impact the patient. Requires the provider to notify the patient or their representative of this belief and to allow inspection of the remaining portion of the records. Permits an independent third party to review the withheld portion of the records to determine whether the records should be withheld until the patient finishes their treatment.
